Indications:
Glimepiride is indicated for the treatment of type II diabetes mellitus in adults, when diet, physical exercise and weight reduction alone are not adequate.

Dosage and administration	Dosage: Dosage should be individualized on the basis of both response and tolerance. Initial dose is 1mg once daily. If adequate glycemic control is not achieved, the dose should be gradually increased at no more than 1–2 mg at 1- to 2-week intervals to 2 mg – 4 mg – 6 mg once daily until blood glucose is controlled. Mamximum dose is 6 mg once daily. Normally, patients respond to doses of 1-4 mg daily. A dosage of more than 4 mg glimepiride per day gives better results only in exceptional cases. In patients with uncontrolled glucose with a maximum daily dose of metformin, the treatment can be combined with glimepiride. Metformin dose is still maintained, glimepiride therapy is started with a low dose, then adjusted gradually until the desired metabolic control is achieved. The combination must be closely monitored by the physician. In patients not adequately controlled with the maximum daily dose of glimepiride, concomitant insulin therapy can be considered. While maintaining the dose of glimepiride, insulin therapy is initiated at low doses and adjusted for the desired level of glycemic control. Coordination with insulin must be closely monitored by the physician. While maintaining the glimepiride dose, insulin treatment is started at a low dose and titrated up depending on the desired level of blood glucose control. The combination therapy should be initiated under close medical supervision. Switch over from other oral hypoglycaemic agents to glimepiride. A switch over from other oral hypoglycaemic agents to glimepiride can generally be done. For the switch over to glimepiride, the strength and half-life of the previous drug must be taken into account. In some cases, especially in antidiabetics with a long half-life (e.g. chlorpropamide), it takes several days for changeover to minimize the risk of hypoglycaemia due to the additive effect. The recommended starting dose is 1mg glimepiride daily. Depending on the patient's response, the dose may be increased gradually as described above. Switch over from insulin to glimepiride. In exceptional cases, where patients with type II diabetes have taken insulin, a changeover to glimepiride may be indicated. The changeover should be closely monitored by physician. Patient with liver failure or kidney failure. There is limited experience available on the use of glimepiride in patients with hepatic or renal insufficiency or patients on hemodialysis. It is recommended that insulin be used in patients with hepatic or renal insufficiency. Children: Data on safety and efficacy in children are inadequate, therefore glimepiride is not recommended. Administration: Glimegim®4 should be taken whole with adequate liquid shortly before breakfast or lunch. Do not chew tablets. The general principle: For initial and long-term treatment, it is necessary to follow your doctor’s instructions very closely. The dosage of glimepiride must be the lowest which is sufficient to achieve the desired blood glucose level. Do not skip meals while taking glimepiride tablets.
Contraindications	Hypersensitivity to glimepiride or to any of the excipients, or to other sulfonylureas, or to sulfonamide derivatives. Insulin– dependent diabetes (type I). Diabetic ketoacidosis. Diabetic coma. Severe renal or hepatic impairment. Pregnant women or breastfeeding mothers.
Warnings and precautions for use	Glimepiride must be taken immediately before or during meals. Patients should be fully instructed on the nature of diabetes and what to do to prevent and detect complications. The goal of treatment is good blood sugar control, so patients have to take the drug regularly, accompanied by diet, appropriate exercise, and weight loss, if necessary. Close supervision is required because hypoglycaemia may occur during initial weeks of the therapy. In case of irregular meals or skipping meals, treatment with glimepiride can lead to hypoglycemia. Common symptoms of hypoglycemia include: Headache, hunger, weakness, sweating, tremor, discomfort, impaired concentration (see Overdose and management). The clinical picture of a severe hypoglycaemic attack may be similar to that of a stroke. Symptoms can almost be controlled quickly by immediate sugar intake. Artificial sweeteners have no effect. Although this measure may initially be effective, but hypoglycemia may recur. Severe hypoglycaemia or prolonged hypoglycaemia, only temporarily controlled by the usual amounts of sugar, requires immediate medical treatment and, in some circumstances, in-patient hospital care. Factors contributing to hypoglycemia include: – Non-compliance with or non-cooperation with drug therapy (commonly seen in the elderly patients). – Malnutrition, irregular mealtimes or skipped meals or being on a diet. – Alterations in diet. – Imbalance between consumption of carbohydrates and physical performance. – Consumption of alcohol, especially in combination with skipped meals. – Impaired renal function. – Severe hepatic dysfunction. – Glimepiride overdosage. – Certain disorders of the endocrine system affect the carbohydrate metabolism or regulation of blood glucose (such as certain disorders of thyroid function, pituitary or adrenocortical insufficiency). – Concurrent administration of certain other medicines (see Interaction with other medicinal products and other forms of interaction). During glimepiride therapy, glucose levels in blood and urine must be monitored regularly. Regular hepatic and hematological monitoring (especially leucocytes and thrombocytes) are required during treatment with glimepiride tablets. In stressful situations (eg, accidents, acute operations, infections with fever etc), a temporary switch to insulin may be indicated. Glimepiride should not be administered to pediatric patients under 18 years of age. Treatment of patients with G6PD deficiency with sulfonylurea agents may lead to hemolytic anemia. Because glimepiride belongs to the sulfonylurea group, caution should be used in patients with G6PD-deficiency and a non-sulfonylurea alternative should be considered. There are limited experiences on the use of glimepiride tablets in patients with impaired hepatic function or kidney disease. In patients with severely impaired renal or hepatic function, insulin is indicated. This drug contains lactose and tartrazine. Lactose: Patients with rare hereditary problems of galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption should not take this medicine. Tartrazine (E102): May cause an allergic reaction.
Recommendation for pregnancy and breastfeeding	Pregnancy: There are no adequate data from the use of glimepiride in pregnant women. Therefore, glimepiride is contraindicated in pregnant women. If patient become pregnant during glimepiride therapy, she must inform the treating doctor to change over to insulin and insulin dosage should be adjusted to keep blood glucose level in a normal range. Lactation: It is not known whether the drug can pass breast milk. Therefore, glimepiride is contraindicated for breastfeeding; Use insulin instead. If glimepiride is required, breastfeeding must be stopped.
Effects on ability to drive and use machines	There is no information about the effects of glimepiride on ability to drive or use machine. However, caution should be exercised when using the drug for the driver or the operator because their ability to concentrate and their response may be reduced due to high or low blood glucose levels and visual impairment. Patients should be advised to take precautions to avoid hypoglycaemia whilst driving.
Interactions, incompatibilities of medicine	Concurrent administration of glimepiride with other drugs may result in increased or decreased effects of glimepiride. Therefore, patient should consult the treating doctor before taking another medicine. Glimepiride is metabolized in the liver by cytochrome P450 (CYP2C9). Its metabolism will be altered by concomitant administration of CYP2C9 inducers (e.g. rifampicin) or with inhibitors (e.g. fluconazole). The following interactions should be noted: Potentiation of the blood-glucose-lowering effect: Hypoglycaemia may occur when glimepiride is used concurrently with following medicinal products: – Phenylbutazone, azapropazone and oxyphenbutazone. – Insulin and oral antidiabetic products, such as metformin. – Antibiotics such as ciprofloxacin, pefloxacin etc. – Salicylates and para-aminosalicylic acid, some nonsteroidal anti-inflammatory drugs (NSAIDs). – Anabolic steroids and male sex hormones. – Chloramphenicol, certain long acting sulfonamides, tetracyclines. – Coumarin anticoagulants. – Fenfluramine. – Fibrates. – ACE inhibitors. – Fluoxetine, MAO-inhibitors. – Allopurinol, probenecid, sulfinpyrazone. – Sympatholytics. – Cyclophosphamide, trophosphamide and iphosphamides. – Miconazole, fluconazole. – Pentoxifylline (high dose parenteral). – Tritoqualine. Weakening of the blood-glucose-lowering effect: Increase in blood glucose levels may occur when one of the following medicinal products is taken: – Estrogen and progestogens. – Saluretics, thiazide diuretics. – Thyroid stimulating agents, glucocorticoids. – Phenothiazine derivatives, chlorpromazine. – Adrenaline and sympathicomimetics. – Nicotinic acid (high dosages) and nicotinic acid derivatives. – Laxatives (long term use). – Phenytoin, diazoxide. – Glucagon, barbiturates and rifampicin. – Acetazolamide. Potentiation or weakening of the blood-glucose-lowering effect: – H2 antagonists. – Beta-blockers, clonidine, guanethidine and reserpine. Alcohol intake may potentiate or weaken the hypoglycaemic action of glimepiride in an unpredictable fashion. Glimepiride may either potentiate or weaken the effects of coumarin derivatives.
Undesirable effects (ADRs)	The most important ADR is hypoglycemia. Common, ADR > 1/100 Neurological: Vertigo, dizziness, headache. Rare, ADR < 1/1000 Blood and lymphatic system disorders: Mild thrombocytopenia, hemolytic anemia, erythropenia, leukopenia, agranulocytosis. Vascular: Allergic vasculitis. Metabolism and nutrition disorders: Hypoglycaemia. These hypoglycaemic reactions mostly occur immediately, may be severe and are not always easy to correct. The occurrence of such reactions depends, as with other hypoglycaemic therapies, on individual factors such as dietary habits and dosage Very rare, ADR < 1/10.000 Immune system disorders: Leukocytoclastic vasculitis, mild hypersensitivity reactions that may develop into serious reactions with dyspnea, fall in blood pressure and sometimes shock. Gastrointestinal disorders: Nausea, vomiting, sensations of pressure or fullness in the epigastrium, abdominal pain and diarrhea. Hepato-biliary disorders: Hepatic function abnormal (e.g. with cholestasis and jaundice), hepatitis and hepatic failure. Investigations: Blood sodium decrease. Not known Blood and lymphatic system disorders: Severe thrombocytopenia with platelet count less than 10, 000/µl and thrombocytopenic purpura. Immune system disorders: Cross-allergenicity with sulfonylureas, sulfonamides or related substances is possible. Visual disorders: Especially at the start of treatment, there may be temporary visual impairment due to the change in blood glucose levels. Hepato-biliary disorders: Hepatic enzymes increased. Skin and subcutaneous tissue disorders: Hypersensitivity reactions of the skin may occur as pruritus, rash, urticaria and photosensitivity. Guidelines for ADR management In general, undesirable effects mentioned above will decrease gradually and resolve themselves during the therapy. If they become severe, the therapy must be discontinued.
Overdose and management	Symptoms: Glimepiride overdose can cause severe hypoglycemia with symptoms: Headache, lethargy, tremor, sweating, clammy skin, restlessness, anxiety, tachycardia, hypertension, palpitation, irritability, chest pain, arrhythmia, ravenous hunger, nausea, vomiting, sleepiness, impaired concentration, alertness and reaction time, speech and visual disorders, paresis, sensory disturbances, dizziness, depression, confusion, loss of consciousness leading to coma. In case of coma, shallow respiration and bradycardia were observed. The clinical picture of a severe hypoglycaemic attack may be similar to that of a stroke. Symptoms may not be present for up to 24 hours after ingestion. Treatment: In case of mild hypoglycemia: Oral administration of glucose or table sugar solution (dissolve about 20 – 30g in a glass of water), and monitoring blood glucose level. Sugar is taken at 15-minute intervals until the blood glucose level are within normal range. If severe hypoglycemic reactions with coma, seizure, or other neurological impairment occur: Patients should be immediately moved to health facilities to be treated with glucagon or concentrated glucose solution. In cases of acute intake of large amounts of glimepiride, detoxification (e.g. by gastric lavage and administration of medicinal charcoal) is indicated.
Pharmacodynamic properties	Glimepiride is a sulphonylurea agent used in the treatment of non-insulin-dependent (type II) diabetes mellitus. Glimepiride acts mainly by stimulating beta-cells of the pancreatic islets of Langerhans to release insulin, thus, it works only if the pancreas remains active (beta cells still produce insulin). Glimepiride acts by binding to receptors in the pancreatic beta cell membrane, leading to closure of the ATP-sensitive potassium channel. Closing the potassium channel induces depolarisation of the beta cell membrane and results – by opening of calcium channels – in an increased influx of calcium into the cell. This leads to insulin release through exocytosis. As with other sulfonylurea hypoglycemics, glimepiride lowers blood glucose levels in both patients with diabetes and in healthy people without diabetes. On long term use, glimepiride also has some extrapancreatic effects, contributing significantly to the hypoglycemic effect of the drug. Among these effects, principal effect is to improve the sensitivity of peripheral tissues for insulin and decrease the production of glucose by the liver. Glimepiride has a synergistic effect with metformin or insulin.
Pharmacokinetic properties	The bioavailability of glimepiride is very high. Food intake does not alter significantly the absorption, only the absorption rate is slightly slower. Maximum plasma concentrations are reached approximately 2.5 hours after oral administration. Glimepiride has a very low distribution volume (approx 8.8 litres) which is roughly equal to the albumin distribution space, high protein binding (>99%), and a low clearance (approx 48 ml/min). The plasma half- life of glimepiride is within 5 – 8 hours, but after high doses, slightly longer half-lives are noted. After a single dose of radiolabelled glimepiride, 58% of the metabolites was recovered in the urine, and 35% in the feces. No unchanged metabolite was detected in the urine. Glimepiride is metabolized in the liver, mainly by CYP2C9. Two metabolites of glimepiride, the hydroxy derivative and the carboxy derivative, were identified both in urine and feces.

Mô tả

Chỉ định:

Điều trị đái tháo đường không phụ thuộc insulin (đái tháo đường typ 2) ở người lớn, khi không kiểm soát được glucose huyết bằng chế độ ăn kiêng, luyện tập thể lực và giảm cân.

Liều dùng và cách dùng:

Cách dùng:

Uống viên thuốc với một ít nước, không nhai, ngay trước bữa ăn sáng hoặc bữa ăn trưa.

Nguyên tắc chung: Điều trị khởi đầu và dài hạn cần theo sự chỉ dẫn của bác sỹ.

Dùng liều thấp nhất đạt được mức đường huyết mong muốn.

Không được bỏ bữa ăn sau khi đã uống thuốc.

Liều dùng : Liều dùng tùy thuộc vào đáp ứng và dung nạp thuốc của người bệnh.

Sau khi bệnh nhân dùng liều khởi đầu: 1mg x 1 lần/ngày, nếu chưa kiểm soát được glucose huyết thì tăng liều từ từ cách nhau 1 – 2 tuần: 2mg – 4mg – 6mg, dùng 1 lần/ ngày cho đến khi kiểm soát được glucose huyết. Liều tối đa là 6mg/ ngày.

Thông thường, người bệnh đáp ứng với liều khoảng 1 – 4mg/ ngày. Liều cao trên 4mg/ ngày chỉ có kết quả tốt hơn ở một số trường hợp đặc biệt.

Ở những người bệnh không kiểm soát được glucose huyết với liều metformin tối đa, thì có thể điều trị phối hợp với glimepirid. Vẫn duy trì liều của metformin, glimepirid được bắt đầu với liều thấp, sau đó được điều chỉnh dần cho tới khi đạt yêu cầu về glucose huyết. Việc phối hợp phải có sự theo dõi chặt chẽ của thầy thuốc.

Ở những người bệnh không kiểm soát được glucose huyết với liều glimepirid tối đa, thì có thể xem xét phối hợp với insulin.Trong khi duy trì liều glimepirid, điều trị bằng insulin được bắt đầu ở liều thấp và được điều chỉnh theo mức độ kiểm soát glucose huyết mong muốn. Việc phối hợp với insulin phải có sự theo dõi chặt chẽ của thầy thuốc.

Chuyển từ các thuốc điều trị đái tháo đường đường uống khác sang glimepirid.

Có thể thực hiện sự chuyển đổi từ các thuốc điều trị đái tháo đường đường uống khác sang glimepirid. Đối với việc chuyển sang dùng glimepirid, cần phải tính đến nồng độ và thời gian bán hủy của thuốc trước. Trong một số trường hợp, đặc biệt là thuốc điều trị đái tháo đường thời gian bán hủy dài (ví dụ clorpropamid), nên cách thời gian vài ngày để giảm thiểu nguy cơ phản ứng hạ đường huyết do tác dụng phụ.

Liều khởi đầu khuyến cáo là 1 mg glimepirid mỗi ngày. Tùy theo đáp ứng của người bệnh, liều của glimepirid có thể tăng dần như đã nêu ở trên.

Chuyển từ insulin sang glimepirid.

Trong những trường hợp ngoại lệ, khi bệnh nhân đái tháo đường typ 2 đã dùng insulin, có thể được chỉ định thay đổi dùng glimepirid. Việc thay đổi phải có sự theo dõi chặt chẽ của thầy thuốc.

Bệnh nhân suy thận hoặc suy gan.

Chưa có kinh nghiệm về việc sử dụng viên glimepirid ở những bệnh nhân suy gan hoặc bệnh nhân thẩm tách máu. Những bệnh nhân suy thận nặng hoặc suy gan nặng khuyến cáo dùng insulin.

Trẻ em: Không đủ dữ liệu về tính an toàn và hiệu quả ở trẻ em, do đó khuyến cáo không dùng thuốc này cho trẻ em.

Chống chỉ định:

Mẫn cảm với glimepirid hay với bất cứ thành phần nào của thuốc, hoặc với các sulfonylurê khác, hoặc với các dẫn chất sulfamid.

Bệnh đái tháo đường phụ thuộc insulin (đái tháo đường typ 1).

Nhiễm toan ceton.

Hôn mê do đái tháo đường.

Suy thận nặng, suy gan nặng.

Phụ nữ có thai hoặc đang cho con bú.

Các trường hợp thận trọng khi dùng thuốc:

Glimepirid phải được uống ngay trước hoặc trong bữa ăn.

Người bệnh cần được hướng dẫn đầy đủ về bản chất của bệnh đái tháo đường và cần phải làm gì để phòng tránh và phát hiện biến chứng. Mục tiêu điều trị là kiểm soát đường huyết tốt, nên bệnh nhân phải uống thuốc đều đặn, đồng thời áp dụng chế độ ăn uống, tập thể dục hợp lý, giảm cân nặng nếu cần. Cần theo dõi sát vì trong các tuần lễ khi mới bắt đầu điều trị, có thể xảy ra hạ đường huyết.

Khi ăn không đúng giờ hoặc bỏ bữa, việc trị liệu với glimepirid có thể dẫn đến hạ đường huyết. Các triệu chứng dễ nhận biết của hạ đường huyết bao gồm: Nhức đầu, đói, yếu, đổ mồ hôi, run, khó chịu, không tập trung (xem thêm mục Triệu chứng quá liều ở cuối phần Hướng dẫn sử dụng thuốc cho cán bộ y tế trong cùng toa này).

Bệnh cảnh lâm sàng của cơn hạ đường huyết nặng có thể giống như một cơn đột quỵ.

Các triệu chứng hầu như có thể được kiểm soát nhanh chóng bởi lượng đường ăn vào ngay lập tức. Chất làm ngọt nhân tạo không có tác dụng. Tuy cách này lúc đầu có thể hiệu quả nhưng hạ đường huyết có thể tái phát.

Hạ đường huyết nặng hoặc hạ đường huyết kéo dài, chỉ được kiểm soát tạm thời bằng cách dùng đường thông thường, cần điều trị y tế ngay và đôi khi phải nằm viện.

Các yếu tố góp phần hạ đường huyết bao gồm:

– Không tuân thủ hoặc không hợp tác điều trị (thường gặp ở bệnh nhân lớn tuổi).

– Suy dinh dưỡng, ăn không đúng giờ hoặc bỏ bữa ăn hoặc ăn kiêng.

– Thay đổi chế độ ăn uống.

– Sự mất cân bằng giữa luyện tập thể lực và lượng carbohydrat ăn vào.

– Uống rượu, đặc biệt là khi kết hợp bỏ bữa ăn.

– Chức năng thận suy giảm.

– Rối loạn chức năng gan nặng.

– Uống thuốc glimepirid quá liều.

Một số rối loạn của hệ thống nội tiết ảnh hưởng đến sự chuyển hóa carbohydrat hoặc điều hòa đường huyết (như một số rối loạn chức năng tuyến giáp, suy thoái tuyến yên hoặc vỏ tuyến thượng thận).

– Uống đồng thời một số loại thuốc khác (xem mục Tương tác của thuốc với các thuốc khác và các loại tương tác khác bên dưới).

Trong khi điều trị, phải đo đều đặn đường huyết và đường niệu. Cần theo dõi thường xuyên về gan và huyết học (đặc biệt là bạch cầu và tiểu cầu).

Trong các tình huống bị stress (ví dụ tai nạn, phẫu thuật, nhiễm khuẩn sốt cao…) có thể chỉ định tạm thời chuyển sang insulin.

Không nên dùng glimepirid cho người nhỏ hơn 18 tuổi.

Điều trị bệnh nhân thiếu G6PD với các thuốc sulfonylurê có thể dẫn đến thiếu máu tan máu. Vì glimepirid thuộc nhóm các thuốc sulfonylurê, cần thận trọng khi sử dụng thuốc này ở những bệnh nhân bị thiếu G6PD và nên xem xét dùng một thuốc không phải sulfonylurê thay thế.

Chưa có kinh nghiệm về việc sử dụng viên glimepirid ở những bệnh nhân suy giảm chức năng gan hoặc bệnh thận. Ở những bệnh nhân suy giảm chức năng thận hoặc gan nặng chỉ định dùng insulin.

Thuốc này có chứa tá dược lactose và phẩm màu tartrazin:

Lactose: Bệnh nhân có vấn đề di truyền hiếm gặp về không dung nạp galactose, thiếu enzym Lapp lactase hoặc kém hấp thu glucose-galactose không nên dùng thuốc này.

Tartrazin (E102): Có thể gây phản ứng dị ứng.

Các khuyến cáo dùng thuốc cho phụ nữ có thai và cho con bú:

Thời kỳ mang thai:

Không có dữ liệu đầy đủ từ việc sử dụng glimepirid ở phụ nữ mang thai. Vì vậy, chống chỉ định dùng glimepirid cho người mang thai. Trường hợp đang điều trị bằng glimepirid, nếu bệnh nhân có thai, phải báo ngay cho thầy thuốc để chuyển sang dùng insulin và phải điều chỉnh liều insulin để giữ glucose huyết ở mức bình thường.

Thời kỳ cho con bú:

Ở phụ nữ cho con bú chưa rõ thuốc có qua được sữa mẹ hay không. Vì vậy, chống chỉ định dùng glimepirid cho người cho con bú; phải dùng insulin để thay thế. Nếu bắt buộc phải dùng glimepirid thì phải ngừng cho con bú.

Tác động của thuốc đến khả năng lái xe và vận hành máy móc:

Chưa thấy thuốc có ảnh hưởng tới người lái xe và vận hành máy móc. Tuy nhiên cần thận trọng khi sử dụng thuốc đối với người lái xe hay vận hành máy vì khả năng tập trung và phản ứng của họ có thể giảm do ảnh hưởng của lượng đường trong máu cao hay thấp và do suy giảm thị lực. Khuyến cáo bệnh nhân thận trọng tránh không để hạ đường huyết trong khi lái xe.

Tương tác của thuốc với các thuốc khác và các loại tương tác khác:

Dùng đồng thời glimepirid với các thuốc khác có thể dẫn đến tăng hoặc giảm tác dụng của glimepirid. Vì lý do này, khi dùng thêm một thuốc khác cần có ý kiến của thầy thuốc.

Glimepirid được chuyển hóa ở gan bởi cytochrom P₄₅₀ (CYP2C9). Chuyển hóa của nó sẽ bị biến đổi trong trường hợp phối hợp với các chất cảm ứng CYP2C9 (ví dụ rifampicin) hoặc với các chất ức chế (thí dụ như fluconazol).

Các tương tác sau đây cần lưu ý:

Tăng tác dụng hạ glucose huyết: Có thể xảy ra tình trạng hạ glucose huyết khi glimepirid được dùng cùng với các thuốc sau:

– Phenylbutazon, azapropazon và oxyphenbutazon.

– Insulin và các thuốc chống đái tháo đường uống, ví dụ như metformin.

– Các kháng sinh như ciprofloxacin, pefloxacin….

– Các salicylat và acid para-aminosalicylic, một số thuốc kháng viêm không steroid (NSAID).

– Các steroid đồng hóa và hormon sinh dục nam.

– Cloramphenicol, một số sulfamid tác dụng dài, các tetracyclin.

– Các thuốc chống đông coumarin.

– Fenfluramin.

– Các fibrat.

– Các chất ức chế enzym chuyển.

– Fluoxetin, các IMAO.

– Alopurinol, probenecid, sulfinpyrazon.

– Các chất ức chế thần kinh giao cảm.

– Cyclophosphamid, trophosphamid và iphosphamid.

– Miconazol, fluconazol.

– Pentoxifyllin (ở liều cao bằng đường tiêm).

– Tritoqualin.

Giảm tác dụng hạ glucose huyết: Có thể xảy ra tình trạng tăng glucose huyết khi glimepirid được dùng cùng với các thuốc sau:

– Estrogen và progestatif.

– Các thuốc lợi tiểu thải muối và lợi tiểu thiazid.

– Các thuốc giống hormon tuyến giáp, glucocorticoid.

– Các dẫn chất phenothiazin, clorpromazin.

– Adrenalin và các thuốc giống thần kinh giao cảm.

– Acid nicotinic (liều cao) và các dẫn chất của acid nicotinic.

– Các thuốc nhuận tràng (dùng dài hạn).

– Phenytoin, diazoxid.

– Glucagon, barbituric và rifampicin.

– Acetazolamid.

Làm tăng hoặc giảm tác dụng hạ glucose huyết:

– Các thuốc đối kháng thụ thể Histamin H2.

– Các thuốc chẹn beta, clonidin, guanethidin, reserpin.

Uống rượu có thể làm tăng hay giảm tác dụng hạ glucose huyết của glimepirid một cách khó đoán trước.

Glimepirid có thể làm tăng hay giảm tác dụng của các thuốc chống đông máu coumarin.

Tác dụng không mong muốn:

ADR quan trọng nhất là hạ glucose huyết.

Thường gặp, ADR > 1/100

Thần kinh: Hoa mắt, chóng mặt, nhức đầu.

Hiếm gặp, ADR < 1/1.000

Rối loạn máu và bạch huyết: Giảm tiểu cầu nhẹ, thiếu máu tan huyết, giảm hồng cầu, giảm bạch cầu, mất bạch cầu hạt.

Mạch: Viêm mạch máu dị ứng.

Rối loạn chuyển hóa và dinh dưỡng: Hạ đường huyết.

Những phản ứng hạ đường huyết hầu hết xảy ra ngay lập tức, có thể nghiêm trọng và không phải lúc nào cũng dễ dàng được điều chỉnh. Sự xuất hiện của các phản ứng như thế phụ thuộc vào các điều trị hạ đường huyết khác nhau, với các yếu tố cá nhân như thói quen ăn kiêng và liều lượng.

Rất hiếm gặp, ADR < 1/10.000

Rối loạn hệ thống miễn dịch: Viêm mạch bạch huyết, dị ứng nhẹ có thể phát triển thành phản ứng nghiêm trọng với khó thở, giảm huyết áp và đôi khi sốc.

Rối loạn tiêu hóa: Buồn nôn, nôn, cảm giác đầy tức ở vùng thượng vị, đau bụng, tiêu chảy.

Rối loạn gan mật: Chức năng gan không bình thường (ví dụ như ứ mật và vàng da), viêm gan và suy gan.

Xét nghiệm: Giảm natri trong máu.

Chưa biết

Rối loạn máu và bạch huyết: Giảm tiểu cầu nặng với số lượng tiểu cầu dưới 10.000 / μl và ban xuất huyết giảm tiểu cầu.

Rối loạn hệ thống miễn dịch: Có thể gây dị ứng chéo với sulfonylurê, sulfonamid hoặc các chất có liên quan.

Rối loạn mắt: Khi bắt đầu dùng thuốc thường có rối loạn thị giác tạm thời do thay đổi về mức glucose huyết.

Rối loạn gan mật: Tăng enzym gan.

Rối loạn da và mô dưới da: Phản ứng quá mẫn của da có thể xảy ra như ngứa, phát ban, nổi mày đay và nhạy cảm với ánh sáng.

Hướng dẫn cách xử trí ADR

Thông thường các ADR nói trên sẽ giảm dần và tự hết trong quá trình điều trị. Trường hợp xảy ra quá nặng thì phải ngừng thuốc.

Quá liều và cách xử trí:

Triệu chứng:

Quá liều glimepirid có thể gây hạ đường huyết với các biểu hiện: Nhức đầu, người mệt lả, run rẩy, vã mồ hôi, da lạnh, lo lắng, nhịp tim nhanh, huyết áp giảm, hồi hộp, bứt rứt, tức ngực, loạn nhịp tim, đói cồn cào, buồn nôn, nôn, buồn ngủ, giảm tập trung, giảm linh hoạt, giảm phản ứng, rối loạn lời nói, rối loạn cảm giác, liệt nhẹ, chóng mặt, rối loạn thị giác, ngủ gà, trầm cảm, lú lẫn, mất tri giác dẫn đến hôn mê. Khi hôn mê thở nông, nhịp tim chậm. Bệnh cảnh lâm sàng của cơn hạ glucose huyết nặng có thể giống như một cơn đột quỵ. Có thể 24 giờ sau khi uống triệu chứng mới xuất hiện.

Xử trí:

Nếu hạ đường huyết nhẹ: Cho uống glucose hoặc đường trắng hòa 20-30g/ cốc nước và theo dõi glucose huyết. Cứ sau 15 phút lại cho uống một lần cho đến khi glucose huyết trở về giới hạn bình thường.

Nếu hạ đường huyết nặng có kèm hôn mê, co giật hoặc tổn thương thần kinh: Cần đưa ngay đến cơ sở y tế điều trị bằng glucagon hay dung dịch glucose ưu trương.

Nếu uống phải những liều lớn: Cần phải giải độc như rửa dạ dày hoặc dùng than hoạt.

Các đặc tính dược lực học:

Glimepirid là một thuốc chống đái tháo đường typ 2 (không phụ thuộc insulin) nhóm sulfonylurê. Tác dụng chủ yếu bằng cách kích thích giải phóng insulin từ các tế bào beta của đảo Langerhans ở tuyến tụy, vì vậy thuốc chỉ có tác dụng khi tụy còn hoạt động (còn khả năng giải phóng insulin).

Cơ chế tác dụng của glimepirid là liên kết với thụ thể ở màng tế bào beta, làm đóng các kênh kali phụ thuộc ATP. Việc đóng kênh kali sẽ gây khử cực màng, làm mở kênh calci, khiến cho ion calci tăng xâm nhập vào bên trong tế bào. Việc tăng nồng độ ion calci nội bào sẽ kích thích sự giải phóng insulin ra khỏi tế bào.

Giống các sulfonylurê chống đái tháo đường khác, glimepirid làm hạ glucose huyết ở người đái tháo đường và cả ở người khỏe mạnh không đái tháo đường.

Khi dùng dài ngày glimepirid còn có một số tác dụng ngoài tụy, góp phần đáng kể vào tác dụng hạ glucose huyết của thuốc. Trong số này tác dụng chính là tăng cường sự nhạy cảm của các mô ngoại vi với insulin và làm giảm sự sản xuất glucose ở gan.

Glimepirid có tác dụng hiệp đồng với metformin hoặc với insulin.

Các đặc tính dược động học:

Glimepirid có sinh khả dụng rất cao. Thức ăn không làm thay đổi đáng kể sự hấp thu của thuốc, nhưng tốc độ hấp thu có chậm hơn. Nồng độ tối đa trong huyết tương đạt được khoảng 2 giờ 30 phút sau khi uống.

Glimepirid có thể tích phân bố rất thấp (khoảng 8,8 lít) tương tự như thể tích phân bố của albumin. Glimepirid liên kết mạnh với protein huyết tương (> 99%) và có độ thanh thải thấp (xấp xỉ 48 ml/phút). Nửa đời trong huyết tương của glimepirid là 5 – 8 giờ, nhưng khi dùng liều cao nửa đời sẽ dài hơn. Sau khi uống glimepirid được đánh dấu, thấy 58% các chất chuyển hóa được thải trừ qua nước tiểu và 35% qua phân. Trong nước tiểu không thấy glimepirid còn ở dạng chưa chuyển hóa. Glimepirid chủ yếu bị chuyển hóa ở gan bởi CYP2C9. Hai dẫn chất hydroxy và carboxy của glimepirid đều thấy trong phân và nước tiểu.
